Possible online services disruption due to Internet related outage

A worldwide technology outage is causing disruption to some State of Illinois online systems.  We are aware of this issue and are diligently working on restoration.

Disaster Impact and Damage Assessment Forms

The Initial Damage Assessment forms and accompanying instructions provided by the Illinois Emergency Management Agency and Office of Homeland Security (IEMA-OHS) can be accessed at the links below. The forms are to be used by local jurisdictions to document both Individual Assistance (IA) and Public Assistance (PA) damage information following a disaster. Please disregard all previous forms as these are the only forms that IEMA-OHS will accept. All comments and questions should be directed to your IEMA-OHS Regional Office.​​

Impact Assessment Form

The Disaster Impact Assessment Form is used to report impacts of the disaster on the local jurisdiction. This form must be submitted with your IA and PA damage assessment forms that document residential and business damages (IA) and/or Category A-G damages (PA).
